What to put into a contract for replacement windows?

I am beginning to start to dream that there is light at the end of the tunnel of selecting replacement windows for my house!!! In my dream I wonder what should be on the contract between me and my selected window company. Instead of them just writing up their typical sales order and me signing whatever they stick in front of me (as long as the price is right), I'm thinking that I ought to put on it somethings of my choosing. And that is where I need your help - what should I have them put into the contract? Below are some ideas of mine (probably some are better than others), what are your suggestions/opinions?

Installed price per window (or per like windows)
NFRC ratings of U-factor, SHGC, VT, air leakage, etc
Manufacturer's model number
Installer's guarantee
Cost and extent of replacement of rotten wood
Cost and extent of capping with PVC aluminum
Install-crew's expertise (like master installer, AAMA, etc)
Date by which windows must be installed
Glass thickness
IG thickness
Thickness and type of glazing/coatings
Fill gas
Clean up of premises
Responsible for breakage of non-window items
No-subcontracting
Grids type and placement and cost
Guarantee and terms of energy bill savings guarantee(companies here promise this)
UV block

Do any of you have a good contract (with terms like the above) that you can either email me or fax me, so that I can incorporate those terms in my contract?

I haven't decided yet. I'm just dreaming of when I do decide. Probably got a few more days to go.

Whatever you decide on. Make sure you check the windows when they arrive. Make sure they have everything you agreed on.

I assure you I am not a dealer, just a homeowner going through all the frustrations as you in trying to decide on the perfect window for my house
